How to Grade Clinical Evidence for Supplements — Buyer's Guide
Not all clinical evidence is equal. Guide to understanding the difference between RCTs, observational studies, animal studies and in vitro research — and what each means for supplements.

The gold standard for supplement evidence is a randomised, double-blind, placebo-controlled trial (RCT) on human subjects at the relevant dose. Below this are observational studies, animal studies and in vitro (cell) research — each progressively less predictive of human outcomes. Always check study type when evaluating supplement claims.
The Clinical Evidence Hierarchy
Grade A — Human RCT on the product: Double-blind, placebo-controlled, randomised trial on the specific supplement (e.g. VigRX Plus®, Semenax® published studies)
Grade B — Human RCTs on key ingredients: Multiple published trials on the specific ingredient at the clinical dose (e.g. KSM-66® at 600mg, Cognizin® at 250mg)
Grade C — Observational studies: Population studies showing associations — not causation. Spermidine longevity studies fall here
Grade D — Animal studies: May suggest mechanisms but cannot predict human outcomes
Grade E — In vitro (cell culture): Demonstrates chemical activity but least predictive of supplement efficacy in humans
The Most Common Misleading Claims
'Clinically proven' without specifying RCT, human subjects or clinical dose
'Studies show' citing animal or in vitro research to claim human benefits
'Traditional use' as a substitute for clinical evidence
'Supports X' when the evidence is for a mechanism, not a human outcome

A randomised, double-blind, placebo-controlled trial (RCT) on human subjects at the relevant dose. VigRX Plus® and Semenax® have product-level RCTs — rare in the supplement industry.
The individual active compounds have been studied in published research — not necessarily the specific supplement product. This is a meaningfully lower standard than a product-level RCT.
Check study type (RCT vs observational vs animal vs in vitro), study population (human vs animal), dose used (matches product label) and source (peer-reviewed journal, not manufacturer press release).
Animal studies suggest mechanisms but frequently do not translate to human outcomes. We do not accept animal study evidence as sufficient for clinical efficacy claims.
Neither participants nor researchers know who received the active supplement vs placebo. This prevents both placebo response and researcher bias from influencing results — the key feature of the most reliable clinical evidence.
